TRANSPORATION POLICY:
Parent must fill out transportation release form for medical treatment, field trips, or after school care. The driver of the van will have the names of the parent’s telephone numbers for each child being transported.
· Children will be loaded and unloaded in and from the vehicle at the curbside
· A child will not be allowed to cross a street unless the child is accompanied by an adult any time before entering or after leaving a vehicle
· All children exiting the vehicle will be accounted for before leaving the vehicle unattended
· A child will never be left unattended in a vehicle
· ALL CHILDREN SHOULD HAVE A SAFETY BELT
· STAFF MUST HAVE LIST OF CHILDREN TRANSPORTED, EMERGENCY MEDICAL TRANSPORT/TREATMENT AUTHORIZATION
· ON BOARD: FIRE EXTINGUISHER, FIRST AID KIT, DRIVER POSSESSING A CURRENT DRIVER’S LICENSE & A COMMUNICATION DEVICE TO COMMUNICATE TO THE CENTER
WATER ACTIVITY POLICY:
Parent will sign permission slip if child can participate in water play. Good Foundations uses a sprinkler during water play. We do not use swimming pools.
FIELD TRIPS:
GOOD FOUNDATIONS will have parent/guardian to sign permissions slip for field trip including permission to transport child. Notification of when and where we will be going will be given, up to at least 48 hours in advance of a field trip.
